Pre-Registration deadline for McKinley's COVID Clinic has been extended! Please submit your Queen's Registration Form and Consent Form (if under 18 years of age) no later than May 13 (Thursday) 5:00pm to mhscovid@mckinleyhs.k12.hi.us. As long as forms are submitted by the deadline, you will be pre-registered and secure an appointment time. There is no maximum capacity limit.
If you have already submitted your forms, you will be receiving a confirmation email and appointment time by Friday morning. Due to the large number of forms being submitted, processing time has been delayed. Thank you for your patience!
Pre-registration paperwork can now be submitted via Google Forms, emailed to mhscovid@mckinleyhs.k12.hi.us or dropped off at the main office.
