McKinley COVID Vaccine & Booster Clinic Thursday 11/4/21

Aloha Tigers,

If you are interested in receiving a 1st dose vaccination, 2nd dose vaccination, or booster shot next week Thursday 11/24/21. Walk-ins will also be accepted on the day of the clinic between 3:00PM - 4:15PM.

Please bring the following:

• Picture ID

• Completed forms

• Insurance card (if applicable, insurance is not required)

• Vaccination card (if returning for a booster)

Below are electronic copies of forms needed for the clinic please fill them out and bring to the clinic.

• Registration Form

• Pre-Screening Form

• Booster Attestation Form (only for those receiving a booster)

• Student Consent Form (only for those under 18 years of age)
